Please use the Order Form below. When you submit your order, the following message will appear in place of the Order Form to show that your order has been transmitted to us:
"Your order has been received. Thank you. When your order is ready to ship, which is usually from on-hand inventory, but normally no more than a maximum of four weeks after receipt of the order, you will be sent a PayPal invoice. Once paid for, your product will then immediately ship by prepaid USPS First Class Mail."
Line items marked with an asterisk in the order form must always be completed, otherwise you will not be able to successfully submit your order to us. Thank you for ordering.

If you are ordering Wholesale and your shipping address is in Georgia, we will need to have a Georgia ST-5 Sales Tax Certificate of Exemption on file for you, otherwise sales tax will have to be charged at the relevant county rate. We will review your order to check this and contact you if necessary.
