Please use the Order Form below. When you submit your order, the following message will appear in place of the Order Form to show that your order has been transmitted to us:
"Your order has been received. Thank you. When your order for Standard wristbands is received you will receive a PayPal invoice for 2/3 of the cost of your order. When that order is ready to ship, which is usually from on-hand inventory, but normally no more than a maximum of four weeks after receipt of order, you will be sent a PayPal invoice for the 1/3 balance of the order. Once paid for, your product will then immediately ship by prepaid USPS Priority Mail. Custom orders will be invoiced on receipt of order and require payment in full at time of order. Custom orders usually ship about six weeks after order, again by prepaid USPS Priority Mail."
Line items marked with an asterisk in the order form must always be completed, otherwise you will not be able to successfully submit your order to us. Thank you for ordering.
