Please use the Order Form below. When you submit your order, the following message will appear in place of the Order Form to show that your order has been transmitted:
"Your order has been received. Thank you. When your order is ready to ship, which is usually from on-hand inventory for standard orders, but normally no more than a maximum of four weeks after receipt of order, you will be sent a PayPal invoice. Once paid for, your product will then immediately ship by prepaid USPS Priority Mail. Custom orders will be invoiced on receipt of order and require payment in full at time of order. Custom orders usually ship about six weeks after order ."
Line items marked with an asterisk in the order form must always be completed, otherwise you will not be able to successfully submit your order to us.
